If so, we are seeking to a highly motivated Health and Wellbeing Assistant to be part of our substance misuse team in Reading.

As a Health and Wellbeing Assistant with Change Grow Live, we treat every person as an individual, so being flexible, empathetic, understanding and a good listener are key. We are looking for an enthusiastic, focused and values driven team player with experience of working in substance misuse.

You will join an experienced team who provide a range of evidenced based interventions for those experiencing difficulties with drugs and/or alcohol underpinned by a genuine commitment to taking a positive and empowering approach to service delivery.

Responsibilities
As directed by the lead Nurse, and Services Manager, the post holder will participate on delivering clinical interventions including titrations, BBV testing and access to treatment, other harm reduction interventions and ensuring that high standards of quality are achieved to deliver key outcomes in line with contractual arrangements and service user needs with a key focus on providing services that are safe, effective, caring, responsive and well led.

Effective and productive working relationships are critical. The post holder will be open minded and committed to effective joint working with key partner agencies to ensure effective communication and collaboration for the benefit of all service users.
