Outreach Mental Health Practitioner

Job Purpose: This role will sit within the CDI Outreach Team who deliver across Borough on site in Community and Educational settings.

Main Duties and Responsibilities:

1. Initiate and build relationships with children, young people and families who come into contact with our service and to create the environment and opportunity for young people to be able to raise any issues or concerns affecting their lives

2. Work integratively as part of the Outreach team of paid and voluntary staff to plan, prepare, deliver (in local housing communities, shopping centres, schools, colleges, leisure centres, parks and youth centres) and evaluate interventions

3. To specifically engage young people with Mental Health-related issues who access the Talkbus as part of the Safe Space/Crisis Intervention initiative alongside NHS and Social Care services that includes individual support, group work and psycho-educational interventions

4. To be part of the Outreach team who are responsible for the delivery of ongoing health education and emotional well-being programmes in response to identified needs within Community and Educational settings

5. The post holder must have a Full UK Manual Driver’s Licence in order to drive and set up the Talkbus with reasonable responsibility for Health & Safety while in transit and on site

6. To keep up to date comprehensive case notes, prepare formal documents, referrals, summaries, collate data, keep accurate records in accordance with CDI Monitoring & Evaluation processes and liaise with the Outreach Administrator to help achieve this and to meet project deadlines. Maintain and record client contact statistics using CDI systems. Ensure administrative tasks are completed within the time scales set down in the service standards

7. To contribute to quarterly and annual reporting back on the work and to contribute to other methods of feedback as required by the Agency.

Job Types: Full-time, Fixed term contract
Contract length: 12 months

Salary: £30,237.00 per year

• Weekend availability
