We are looking for a Support Worker to join The Community Mental Health Team for Older People. The service consists of two community mental health teams and has close links with the care home team, post diagnostic service and inpatient services.

 

This is an opportunity to become part of an integrated multidisciplinary team providing a high standard of therapeutic input for people with mental health needs living in the community. The team consists of nursing staff, social workers, occupational therapists, support staff, psychologists and medical staff. The post will involve liaising with other statutory and non statutory organisations. Successful applicants will assist in the delivery of individualised care under the supervision of qualified staff. The post involves planning and delivering care for individuals in their own homes in conjunction with other members of the multidisciplinary team.

Previous experience of working with older people with mental health needs and good communication skills are essential.
