Job Description

Picton Primary Care Network (PCN) are looking for a Care Coordinator to join the team and provide excellent patient care across a number of our largest programmes that include; Cancer, Women’s Health and our Care Homes & Housebound service.

This role will report into the Lead Care Coordinator and be responsible for taking charge of one of our key programmes of work.

Daily duties:

  • Organise and manage the ROTA and appointments of the relevant clinical teams to support them in delivering excellent patient care.
  • Demonstrating creativity and innovation in improving our current services by creating new ways that we can engage with our patient population.
  • Deliver health engagement events in our community with the support of various partners.
  • Deliver reports and audits to Leadership board demonstrating effectiveness of the service.

About Picton PCN

Picton PCN consists of 6 GP practices across central and south Liverpool that support 50,000 patients. Our PCN team consists of 6 Care Coordinators, 5 Pharmacists, 3 Paramedics, 2 Physios, 3 Link Workers and a Mental Health team of 7 clinicians.
