HEALTH AND WELLBEING TEAM MANAGER, BERKSHIRE WEST

HOURS: * 28 – 37 hours per week (full or part-time)
SALARY: £29,757 – £33,454 per annum (SCP 24-28), pro-rata
CONTRACT: Permanent
HOLIDAYS: 28 days plus bank holidays
PENSION: Auto enrolment in place
PROBATIONARY PERIOD: 6 months

This exciting new role will be at the forefront of implementing two new projects: A&E Outreach Support and Physical Health Support Pilot.

We are seeking a motivated individual with strong understanding of the impact of mental health illness on physical and mental health. The ability to work in partnership and seek the best outcomes for service users is key to the post. The postholder will need to be able to support workers to complete challenging work and effectively manage risk.

ABOUT THE ROLE AND CANDIDATE*

PURPOSE OF THE JOB*
The post holder will work to set up and establish 2 exciting new Wellbeing projects, working under the supervision of a Wellbeing Service Manager and in partnership with NHS clinical services.

The post will be responsible for overseeing:

A&E Outreach Support Project: *
This project is delivered in partnership with Berkshire Health Foundation Trust and the Psychological Medicine Service (PMS) based within Royal Berkshire Hospital A&E department. The workers will aim to support people who have attended A&E in mental health crisis, with suicidal ideation or self harm and who are due to be discharged back to Primary Care.

Physical Health Support Worker Pilot*
This project is to be delivered in partnership with Primary Care Networks in Wokingham Borough. Workers will engage with people on the Severe Mental Illness (SMI) register to ensure that they receive a physical health check, and support and advice on accessing services to support their mental health.

Key responsibilities
*

  • Implement projects in partnership with key stakeholders, to agreed timelines.
  • Line manage Wellbeing Workers and provide support, advice and guidance to staff making day-to-day decisions.
  • Maintain awareness of and support workers to implement relevant aspects of best practice in mental health crisis support and holistic support for physical health in relation to Severe Mental Illness (SMI).
  • Oversee the recruitment and induction of key staff.
  • Manage and analyse the monitoring information collected from projects, reporting regularly to funders and Oxfordshire Mind/Mind in Berkshire Senior Management Team.
  • Establish excellent working relationships with staff in partner agencies such as Royal Berkshire A&E, Psychological Medicine Service, Primary Care, Secondary Care Teams, and Voluntary and Community Organisations for the benefit of service users and professionals.
  • Raise awareness of the projects locally and ensure the service is promoted to relevant professionals and members of the public.
  • Support Wellbeing Workers to effectively assess and manage risks identified when supporting service users and work closely with clinicians to ensure appropriate mitigations are in place.

PERSON SPECIFICATION / SELECTION CRITERIA*

Essential Experience
a) Experience of managing or coordinating a project/service within mental health or similar services
b) Experience of partnership working in service delivery in a leadership role.
c) Experience of data collection and reporting to measure outcomes and service impact

Skills and Knowledge
d) Excellent organisational and team management skills and a high level of ability to support and supervise staff.
e) Knowledge and understanding of mental health issues, mental health services and the needs of people living with mental health problems.
f) Understanding of the links between physical and mental health, and the impact of severe mental illness on physical health.
g) Ability to respond effectively to challenging situations and take appropriate action to manage risk or safeguarding concerns.
h) Capacity to cope effectively with the pressures of the post and to use the support and supervision provided.
i) Strong commitment to the involvement of people who use services, social inclusion, recovery and partnership working.
