Paragon Home Healthcare are seeking a Nurse Practitioner registered with the Nursing and Midwifery Council (NMC). We are a leading provider of comprehensive home care services, uniquely specialised for individuals with spinal cord injuries and those requiring complex care. Endorsed by the Spinal Injuries Association as their trusted provider, we are dedicated to enhancing accessibility and community integration for our clients.
In this role, you will provide clinical advice and instruction, troubleshoot clinical concerns, and manage medications. You will contribute to the safe and effective care of patients who receive complex and specialised home care, such as those with spinal cord injuries requiring 24-hour care. These clients are cared for by specially trained carers in their homes.
Responsibilities:
- Triage and Clinical Advice: Manage triage checks and provide clinical advice based on incoming clinical requests, including medication queries and deliveries not recorded in the care planning software.
- Medication Management: Transcribe medications onto the care planning software, make adaptations and amendments, add new medications, and stop medications as per the prescriber’s prescription.
- Review of Care Notes: Regularly review care notes to identify risks and ensure that all care meets the required standards.
- Record Keeping: Maintain detailed records for audit, contracting, and care records, ensuring accuracy and up-to-date documentation.
- Quality and Safety Checks: Audit notes and perform quality and safety checks on accident forms, incidents, clinical requests, skin forms, medication changes, and general clinical advice.
- Writing and Auditing Care Plans: Write, audit, and review care plans and risk assessments to ensure they are fit for purpose.
- Consultation and Referrals: Provide consultations, signpost clients to appropriate services, and make necessary referrals.
- Availability: Offer clinical advice and safeguards during working hours and participate in an on-call advice system outside business hours.
- Liaison and Recommendations: Communicate with GPs, consultants, and specialist nurses to make recommendations and liaise closely with clinical and operations directors and the senior management team for escalations.
- Enhancing Clinical Practices and Care Quality: Update procedures, identify improvement areas, and develop strategies to enhance clinical practices and care quality.
- Professional Development: Ensure your practice is up to date with all relevant training and skills.
- Care Management: Contribute to the safe and effective care management for clients, maintaining high standards of care.
Requirements:
- NMC Registration: Must be a registered nurse (adults) with an active NMC pin.
- Experience: Confident in triaging and troubleshooting care issues that require a registered healthcare professional.
- Clinical Skills: Competent in managing clinical queries, medication transcriptions, and making necessary adaptations to care plans.
- Background: Ideally, a nurse practitioner or someone with experience in a similar consulting role, such as in a GP surgery, walk-in centre, or providing advice for services like the NHS 111 service. Experience with medication prescribing or transcribing is a significant advantage.
- Communication: Strong communication skills to effectively manage clinical requests and liaise with carers and other healthcare professionals.
- Problem-Solving: Ability to make informed decisions and take responsibility for client care and referrals.
- Self-Directed Work: Comfortable with self-directed work and managing a varied workload.

Salary: £40,000 – £60,000 pa depending on experience.
