An exciting opportunity has arisen for a part time Practice Nurse to join our established Practice and Nursing Team to work up to 34 hours per week, Monday to Friday, but with flexibility to cover annual leave and sickness when required.

About us

Our team consists of 2 GP Partners, a long-term locum GP, a Pharmacist, Nurse Practitioner, Practice Nurse, Nursing Associate and a Health Care Assistant and a supportive Management, Reception and Administrative team.

South King Street is part of Central West Primary Care Network who employ a Multidisciplinary Team consisting of Occupational Therapist, Pharmacists, Pharmacy Technicians, Care Coordinators and Social Prescribers that provide an invaluable contribution to the care of our patient population.

Job description

Chronic Disease Management:

1. Undertake clinical review and provide holistic care in line with practice protocols for patients with the following conditions:

  • Asthma
  • COPD
  • Diabetes
  • IHD
  • Hypertension
  • CKD

2. Assist with the appropriate recall for all CDM patients.

3. Assist and work with the practice team to ensure all clinical targets are achieved each year on a CDM register.

Vaccination and immunisation

General Principles:

1. Ensure all vaccines are administered under individual prescriptions or ratified patient group directions.

2. Obtain and document that valid consent has been obtained from the patient, parent or legal guardian prior to each and every vaccination.

3. Administer the correct vaccine(s) by the correct route, at the correct site, at the correct time.

4. Safely dispose of used vaccines and sharps according to national local infection control guidelines.

5. Ensure appropriate advice and follow up, is given and documented.

6. Have knowledge of the appropriate information to be recorded and ensure documentation is completed as per patient group directions and professional guidelines.

7. Be competent in the recognition and management of anaphylaxis as per current guidelines.

8. Ensure patients/parents/guardians are informed of the management of potential side effects of the vaccines to be administered, (verbal and written).

Childhood Immunisations

Undertake childhood immunisation clinics alongside another health care professional working to the national programme. Adhering the general principles above.

General Treatment Room Duties

1. Perform Blood Pressure Checks.

2. Perform Venepuncture.

3. Undertake ECG recordings where applicable as requested.

4. Implement 24-hour Blood Pressure recording as requested.

5. Offer basic lifestyle advice on smoking, diet, exercise, and alcohol.

6. Identify and offer appropriate support to enable patients to make positive behaviour changes to improve their lifestyle.

7. Measure and record Body Mass Index.

8. Undertake Health Checks.

9. Prepare, support and chaperone patients who require intimate examinations.

10. Carry out routine urinalysis assessments.

11. Interpret and review blood results.

Wound Management

1. Use a holistic approach to assess, manage, and implement wound care in line with current evidence-based practice and local guidelines.

2. Suture removal

Cytology and Contraception if qualified

1. Undertake cervical cytology, maintain professional competency on an annual basis.

2. Provide routine contraception care in line with practice protocols.

Emergency Management

1. Ensure the treatment rooms are maintained in line with local infection control and health and safety guidelines.

2. Assist with the monitoring and management of stock and equipment to include:

Daily assessment and recording of minimum and maximum refrigerator temperature.

Assess and record emergency equipment checks.

Stock control of equipment, dressings, vaccines, drugs, and clinical products.

Clinical Leadership

1. Contribute towards practices policies and protocols.

2. Work effectively in the team.

3. Identify personal development and training needs.

4. Provide support and mentoring.

5. Assist with medical and nursing student training.

6. Act as a lead within the safeguarding team and be a focus for external contacts to share information that may be required for safeguarding reviews, domestic homicide reviews, multi-agency/individual agency reviews and contributions to safeguarding investigation where appropriate.

Experience

Essential

  • Ability to communicate effectively within the primary care team and to maintain clear and effective records of patient contacts.
  • Ability to work effectively as part of a team and under own initiative with minimal supervision.
  • Ability to empathise and reassure.
  • Ability to organise and prioritise workload.
  • The post holder must be able to meet the physical and clinical demands of the job description.
  • Ability to work within approved clinical guideline and protocols and an awareness of when to refer patients for further clinical assessment.
  • The post holder should be able to contribute constructively to relevant policy developments within the practice.
  • Ability to participate in clinical audits.
  • Ability to assist the GP in the performance of minor surgical procedures.
  • Ability to cope with distressing or emotional situations.
  • Knowledge of needs of patients with long-term conditions.
  • Awareness of accountability of own role and other roles in a nurse-led service.
  • Awareness of clinical governance.
  • Knowledge of patient group directions.
  • Immunisations and Vaccinations.

Qualifications

Essential

  • Fully registered RCN nurse
  • Prior experience of working within general practice.
  • Experience of monitoring and advising patients with chronic disease.

Desirable

  • Independent prescribing
