Job Description

About the role

We have an exciting opportunity to recruit a  Senior Health Liaison Social Worker to join our Locality Team at the London Borough of Merton. Merton is an outer London borough at the start of the Northern Line and has excellent transport links. The borough includes the areas of Wimbledon, Mitcham, and Morden.

This is a great time to join one of our dynamic teams as we build and consolidate good practice across the borough.  Indeed, the London Borough of Merton is on a journey to excellence. Therefore, we are building on the changes and improvements Merton has been making, ensuring that we work in an outcomes – focussed way whilst focusing on interventions that promote prevention / early intervention.

 About You

We are hoping to recruit an enthusiastic, compassionate, positive thinking and self-motivated individual who has a desire to support people to maximise their independence. The vision is that the social worker would develop constructive working relationships with community / health colleagues, particularly G.Ps, and take the lead in promoting new ways of working to ensure integrated and targeted case management, and increased self-management of health / social care conditions. Thus, preventing hospital and care home admissions.

We are looking for an experienced and registered social worker who is keen to support people in our local area, and carry a complex caseload.  In this role, you will be working with a vast client group, with a focus on health needs. The role involves undertaking case management duties, under the advice and guidance of the Team Manager, and carrying out the statutory duties of the Local Authority. Experience in either Adult’s Services or a multi-disciplinary setting would be desirable, and you will need to be able to demonstrate your knowledge and understanding of the Care Act assessment process, and other relevant Adult Social Care Legislation. We are also very proud to be a member of the Developing Together Social Work Teaching Partnership, meaning we work collaboratively with Kingston University, seven other local authorities and two voluntary sector agencies across South West London & Surrey. This offers social workers a host of unique benefits, including access to a comprehensive learning and development programme that complements Merton’s internal offer and fantastic opportunities for connecting with peers and sharing best practice.

What we offer

We are an equal opportunity employer, and our policies and practices ensure that no person will be discriminated against for reasons of race, gender, disability, sexual orientation, pregnancy or maternity, religion or belief, age, gender reassignment, or marriage and civil partnership.

You’ll also get the training and development you need to excel in your career, as well as opportunities to move between departments and to progress into more senior roles.
