About the role

We have an exciting opportunity to recruit a  Senior Health Liaison Social Worker to join our Locality Team at the London Borough of Merton. Merton is an outer London borough at the start of the Northern Line and has excellent transport links. The borough includes the areas of Wimbledon, Mitcham, and Morden.

 About You

We are hoping to recruit an enthusiastic, compassionate, positive thinking and self-motivated individual who has a desire to support people to maximise their independence. The vision is that the social worker would develop constructive working relationships with community / health colleagues, particularly G.Ps, and take the lead in promoting new ways of working to ensure integrated and targeted case management, and increased self-management of health / social care conditions. Thus, preventing hospital and care home admissions.

We are looking for an experienced and registered social worker who is keen to support people in our local area, and carry a complex caseload.  In this role, you will be working with a vast client group, with a focus on health needs. The role involves undertaking case management duties, under the advice and guidance of the Team Manager, and carrying out the statutory duties of the Local Authority. Experience in either Adult’s Services or a multi-disciplinary setting would be desirable, and you will need to be able to demonstrate your knowledge and understanding of the Care Act assessment process, and other relevant Adult Social Care Legislation.
